Position Details Grade: 14 Location: MDH Wicomico County Health Department, 108 East Main Street, Salisbury, MD 21801 Main Purpose of Job The main purpose of this position is to provide Coordination of Community Services (CCS) to eligible individuals with Developmental Disabilities who reside in Wicomico County. Minimum Qualifications Education: Graduation from an accredited high school or possession of a high school equivalency certificate. Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources. Notes: Candidates may substitute 30 college credit hours from an accredited college or university for the required experience. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education. Desired or Preferred Qualifications Experience providing case management services using a person centered approach. Experience attending and facilitating team meetings for assigned individuals. Experience conducting and completing person centered plans and monitoring meetings. Benefits
