Main Purpose of Job This position will provide targeted case management services to eligible individuals with Developmental Disabilities who live in Cecil, Harford, and Kent Counties. It involves referring for services, facilitating the development of the annual Person‑Centered Plan, monitoring individuals in service and on the Eligibility Waiting List, and assisting with transition planning. The role requires a highly organized, detail‑oriented candidate who helps maintain community participation and promote individual choice to enhance overall quality of life. Minimum Qualifications Education: Graduation from an accredited high school or possession of a high school equivalency certificate. Experience: One year of professional experience in coordinating a health‑related program or referring people to governmental and private resources. Notes Candidates may substitute 30 college credit hours from an accredited college or university for the required experience. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health‑related field of work on a year‑for‑year basis for the required experience and education. Desired or Preferred Qualifications Experience multitasking and meeting multiple deadlines. Experience working with the developmentally disabled population.
