Physical Therapist - Home Health

Schedule: 8-hour day shift

Assignment Details: Patient Ratio/Workload: 1:1; may visit up to 6 patients per day

Requirements: Licensure: OR and WA licensure required (pending license accepted); Minimum 1 year skilled home health experience required; OASIS experience required; Required: BCLS certification; Charting systems: EPIC Remote Client for Home Health, Rover app

Roles & Responsibilities: Provide one-on-one rehabilitation therapy to patients in their homes; Assess and develop individualized treatment plans based on patient needs; Utilize knowledge in orthopedic and neurologic rehabilitation; Educate patients and families on home exercise programs and proper body mechanics; Document patient progress and treatment outcomes in compliance with OASIS standards

Compliance / Notes: References: 2 required covering 1 year within the last 3 years; Submission items requested: list any time-off requests, confirm interview times, confirm traveler phone number; Pre-start modules: Average ~16 hours (non-billable); >20 hours requires PM approval
