Implement individualized treatment programs, including therapeutic exercises, gait training, functional mobility, and restorative modalities. Reassess progress, document outcomes in the care plan, and adjust goals as needed in collaboration with the interdisciplinary team. Educate residents and families on home exercise programs, safety, and discharge planning. Communicate effectively with nurses, physicians, and therapy colleagues to optimize resident outcomes and ensure regulatory compliance.
