Job Description
Title : Physical Therapist

Schedule : 8 hours

Assignment Details :
- Contract Length: 14 weeks
- Guaranteed Hours: 40

Requirements :
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an accredited program - Required

Roles & Responsibilities :
- Conduct clinical assessments to determine patient needs and treatment appropriateness.
- Document and maintain comprehensive patient care plans and progress notes; revise plans based on ongoing assessments.
- Provide treatment for neuromuscular, neurological, and orthopedic disorders according to medical referrals.
- Educate patients and families on therapeutic procedures and correct equipment usage.
- Supervise other team members in providing care consistent with patient needs and desires.
- Apply infection control measures during patient care activities.
