Community Physical was founded in 1986 by Bob Tripicchio, PT, MS, DSc, on the premise that people are the most important asset of any organization. CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services. Physical Therapist Floater – Full-Time (Home Care & Outpatient) $48.00–$52.00 per hour Full-Time | Monday–Friday | 40 Hours per Week | No Weekends Join Community Physical Therapy's Continuum of Care Program, where you'll have the unique opportunity to provide both home care and outpatient physical therapy services within Alden's Assisted Living and Independent Living communities. This role allows you to build lasting relationships with your patients by following them throughout their rehabilitation journey, delivering seamless, high-quality care in both the home and outpatient settings.

What You'll Do

• Treat residents within Alden Assisted Living and Independent Living communities
• Deliver individualized, patient-centered care throughout the continuum
• Collaborate with an experienced interdisciplinary therapy team
• Document: Home care services in Homecare Homebase Outpatient services in TheraOffice
