Full or Part-time: Beaver Falls & surrounding areas The Licensed Practical Nurse (LPN) works under the guidance of the Registered Nurse (RN) as a member of a team to give direct nursing care to patients requiring intermittent visits. The LPN collaborates patient care with the case managing RN, physician, and patient and follows a patient specific treatment plan.

QUALIFICATIONS

Graduate of state-approved school with course in pharmacology Current license in the State of Pennsylvania Minimum of two [2] years experience in patient care Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state

DUTIES AND RESPONSIBILITIES

Clinical Skills Provides skilled nursing care that includes activities such as teaching and treatments (including IV therapy- if IV certified, venipuncture and complex wound care) and health promotion practice Executes preventive, rehabilitation and restorative nursing procedures appropriate for the patient’s care and safety Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, changes in patient’s physical, mental or emotional condition to the physician and the supervising Registered Nurse Evaluates the patient and home situation to determine the health teaching required at each skilled visit. Instructs and educates the patient and the designated representative regarding the care requirements of the patient in the home. Conducts a comprehensive physical examination and nursing assessment of the patient at each skilled visit. Documentation Tablet is synced within 24 hours.
