Description The practice of nursing as a Licensed Practical Nurse is defined as performing tasks and responsibilities within the scope of supportive and restorative care, medication administration, and specialized tasks under the direction of a Registered Professional Nurse, licensed or otherwise legally authorized physician or dentist. Responsibilities Clinical/Technical/Service Participates in the application of the nursing process to an assigned group of patients. Demonstrates behaviors reflective of the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse. Performs job duties under supervision of a Registered Professional Nurse. Demonstrates a competent level of approved skilled nursing care tasks and procedures, including vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only, urinary catheter insertion and care, simple dressings, and removal of a peripheral line. Administers IV fluids, upon completion of annual competency IV therapy training. Monitors patients receiving blood, blood products, and TPN under RN direction. Provides recommendations in the application of the nursing process. Participates in planning and administers direct patient care, including activities of daily living. Performs point‑of‑care testing, including blood glucose. Performs nursing procedures as defined in the nursing procedure manual. Administers medication as defined in nursing policy; all routes except IV push/bolus (unless a normal saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. LPNs cannot administer the first dose of an IV drug; the RN administers the first dose and assesses for side effects, after which the LPN can give subsequent doses. May, under direct RN supervision and after completing an approved hemodialysis training and competency program, perform dialysis‑related tasks as permitted by NYSED Office of the Professions and 10 NYCRR §700.4(b). This does not permit independent practice or unsupervised access to central venous lines. Assists in patient assessment after RN has completed the initial patient assessment, including pain management. Carries out medical and nursing orders; cannot accept verbal orders or carry out RN standing orders. Makes, reports, and records observations on assigned patients within the EMR. Observes and reports any signs and symptoms of change in a patient’s condition to the appropriate person. Participates in health teaching to patients and families following RN development of the teaching plan. Establishes an environment that promotes optimum physical and emotional comfort. Performs purposeful hourly care checks and supports the patient experience in accordance with the Relationship‑Centered Care Professional Practice Model. Organizes supplies and equipment necessary for carrying out the plan of care identified and communicated by the charge nurse or designee. Contributes specified data to the patient’s medical record. Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served in the assigned area. Age‑specific competencies: Identifies physical, behavioral, and emotional characteristics typical for the age group. Modifies approaches based on patient age‑specific needs and responses to treatment. Provides care based on age‑specific needs. Uses communication techniques which are age‑appropriate. Demonstrates awareness of the goals of the Nursing Department. Supports the patient experience to achieve safe and quality care. Adaptation to changing departmental needs reflects awareness of institutional needs. Time management meets the needs of the unit or department. Interprets hospital policies and regulatory standards of care. Recommends changes in nursing practice or policy through the appropriate leadership or nursing committee. Organizational/Managerial Participates in the Nursing Department Quality Improvement Program and activities. Maintains patient and employee confidentiality in the management of information. Educational/Professional Development Participates in the development of other staff members. Meets regulatory, licensure, and annual health assessment requirements. Participates in patient care conferences. Maintains nursing care standards through workshops, in‑service programs, and staff meetings. Maintains competence through continuing education and other appropriate experiences. Communication/Relationships Demonstrates a professional, courteous, and respectful attitude toward patients, families, and significant others. Displays courtesy, professionalism, and respect during interactions with all members of the hospital staff and extended community. Responsible to the leadership person of the unit. Provides direction, assistance, and support to auxiliary staff. Participates in interdepartmental and intergroup communication to provide continuity of patient care. Participates in departmental committees as assigned. Adheres to MSHS legal and ethical standards. May be required to perform other duties as assigned. LPN in the Outpatient Hemodialysis Setting An LPN can practice in the chronic outpatient hemodialysis department under direct supervision of an RN, participate in educational programs, and provide care for patients with central venous access devices. Responsibilities include: Administering a bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first‑dose administration which is performed by an RN. Identifying and setting up supplies necessary for accessing a CVAD. Initiating treatment with a CVAD. Drawing blood specimens from a CVAD. Performing CVAD dressing changes and exit‑site care. Monitoring and adjusting flow rates of a CVAD. Intervening if there is a catheter/blood line disconnection or other complications associated with a CVAD. Providing CVAD care at the end of dialysis treatment. Qualifications Education Requirements Completion of an educational program for licensure as an LPN. Successful completion of the New York State Department of Health LPN IV Therapy Initial Program and the annual unit‑based review of intravenous therapy procedures. Successful completion of the annual unit‑based review of procedures associated with intravenous therapy. Experience Requirements Appropriate experience in a health care setting preferred.
