Scheduling needs may vary based on business demands, and adjustments may be required as reasonably assigned.\n\nCOMPENSATION RANGE: Compensation ranges from $50,000 per year to $167,560 per year, paid on a fee-for-service basis for servicing assigned clients as scheduled, and is commensurate with credentials and experience. Candidates who choose the fee-for-service compensation model may work hybrid.\nThis position may also be paid on an hourly basis, ranging from $25 per hour to $45 per hour, for servicing assigned clients as scheduled. (Legal Information Institute)\n\nSUMMARY OF POSITION RESPONSIBILITIES\nThe Treatment Coordinator role is a key position to ensure that evaluation, treatment planning, support services, collaboration, and discharge processes comply with OMHC standards, including evaluative services, treatment planning and documentation, support services, and discharge/continuity-of-care expectations. \nThe Treatment Coordinator is a licensed behavioral health professional responsible for:\n\nProviding structured, weekly, face-to-face clinical sessions to an assigned caseload of approximately 25 clients\nLeading and coordinating all aspects of those clients’ treatment, including referrals, collaboration, and care transitions\nConducting structured weekly audits of treatment and documentation for assigned clients and their treatment teams\nEnsuring that services for those clients fully conform to their Individual Treatment Plans (ITPs) and to COMAR requirements\n\nThis is a full-time, in-office position that can be hybrid work depending on chosen compensation model. The schedule is structured to support a consistent service model:\n\nMorning: See 5 of the 25 assigned clients each day (rotating so all are seen weekly) in 45-minute face-to-face sessions. These sessions collectively make up 15 hours per week of direct client intervention.\nAfternoon: Provide care coordination, documentation, treatment planning, team leadership, and follow-up for the clients seen that morning and others on the caseload, totaling 25 hours per week of coordination and administrative clinical work.\n\nThe Treatment Coordinator functions as the Team Lead for the assigned client caseload and associated treatment team members.\n\nESSENTIAL DUTIES & RESPONSIBILITIES\n1. Clinical Evaluation & Assessment\n(Aligned with COMAR 10.21.20.06 – Evaluative Services Provided) (Legal Information Institute)\n\nComplete, or ensure completion of, an initial face-to-face diagnostic evaluation and related documentation for each assigned client in accordance with OMHC policy and within required timelines (no later than the second visit).\nFormulate and document: presenting problem(s), relevant psychosocial and family history, mental status, risk factors, strengths, functional impairments, and current needs.\nWhen clinically indicated and with proper consent, maintain and document ongoing collaboration with the individual’s primary care provider or other somatic providers to support integrated care. (MdRules)\nUpdate evaluative information as clinical status changes or at required review intervals.\n\n2. Treatment Planning & Documentation\n(Aligned with COMAR 10.21.20.07 – Treatment Planning and Documentation) (Legal Information Institute)\n\nEnsure that an Initial Individual Treatment Plan (ITP) is developed no later than the fifth visit following enrollment, using the assessment as the foundation.\nDevelop ITPs in collaboration with the client, and when applicable, the client’s parent/guardian/caregiver, and other members of the treatment team.\nEnsure that each ITP includes at minimum:\n\nCurrent diagnosis\nPresenting needs, strengths, recovery goals, and treatment expectations\nSpecific services and interventions (modality, frequency, duration, and responsible staff)\nHow services will help the individual manage symptoms and support recovery\nShort-term and long-term goals written in behavioral, measurable terms with target dates\nIdentification of needed referrals and collaboration with other providers or agencies (e.g., PRP, housing, self-help, vocational, substance use services, schools, social services, juvenile services, somatic care). (Legal Information Institute)\n\nConduct monthly treatment plan reviews with each client, revising goals and interventions based on progress, new information, audit findings, or changes in clinical necessity.\nEnsure documentation is completed accurately, thoroughly, and within required timeframes for all services provided.\n\n3. Direct Client Intervention (15 Hours per Week)\n\nMaintain a caseload of approximately 25 clients at any given time.\nFollow the service model of seeing 5 clients each morning (Monday–Friday) for 45-minute face-to-face sessions, such that each client is seen weekly, barring cancellations or clinically indicated frequency changes.\nDuring sessions:\n\nReview treatment progress and the client’s treatment experience\nAddress clinical and practical barriers to engagement\nIntervene to support treatment compliance, including motivational enhancement, skill-building, and problem-solving\nEducate clients about risks and implications of going against medical or clinical advice (AMA), and document such education\nBegin or support safety planning when indicated\n\nAddress client concerns and complaints related to services, and escalate appropriately when needed.\n\n4. Treatment Coordination & Referral Management (25 Hours per Week)\nAfternoon work is dedicated to coordination, oversight, and administrative clinical duties for clients on the caseload, especially those seen that day.\n\nCoordinate all aspects of services as described in the ITP, ensuring that services delivered match what is authorized, clinically indicated, and documented.\nCommunicate regularly with psychiatrists, nurse practitioners, therapists, PRP staff, case managers, and other providers on the treatment team.\nIssue and track referrals to internal and external resources, within scope, such as:\n\nPsychiatric rehabilitation / PRP services\nSubstance use/addictions treatment\nHousing and shelter programs\nVocational/educational services\nSomatic/primary care providers\nSocial services, juvenile services, and other community supports\n\nEnsure referrals and support services follow the expectations for support services for OMHCs. (Legal Information Institute)\nMonitor and document somatic status information in the medical record (past/current medical history, relevant somatic conditions, provider information) in line with OMHC regulations on evaluation and planning services.(Legal Information Institute)\nIdentify and troubleshoot systemic and individual barriers (transportation, insurance, communication, etc.) and collaborate with internal staff and external agencies to keep care on track.\nParticipate actively in efforts to retain clients in treatment as long as there is clinical necessity, while planning for step-down or transition when appropriate.\n\n5. Weekly Treatment & Team Audits (Quality Oversight)\n\nConduct a structured weekly audit of treatment for each client on the assigned caseload, focusing on:\n\nAlignment between services delivered and the ITP\nFrequency and duration of services compared to what is planned/authorized\nGoal progress, continued clinical necessity, and discharge/transition planning\n\nReview the work of assigned treatment team members (e.g., therapists, PRP staff, case managers) for:\n\nCompleteness and timeliness of documentation\nClinical appropriateness and adherence to the ITP\nCompliance with OMHC, COMAR, payer, and agency documentation standards\n\nIdentify areas of non-alignment or non-compliance and implement corrective actions (e.g., treatment plan revision, staff coaching, scheduling adjustments).\nUpdate treatment plans as needed based on audit findings, clinical progress, or changes in need.\nDocument audit activities, findings, and follow-up in the client record and/or quality management tools as required.\n\n6. Intake, Orientation & Discharge\n\nParticipate in or conduct intake evaluations for clients who will be part of the Treatment Coordinator’s caseload, including orientation to services and review of rights and expectations.\nParticipate in discharge planning and continuity-of-care processes in accordance with COMAR expectations, including ensuring a documented discharge summary within required timelines that includes reason for admission, reason for discharge, services provided, progress made, diagnosis at discharge, current medications, and follow-up recommendations. (Legal Information Institute)\nPromote smooth transitions to higher or lower levels of care, or to other providers/agencies, when medically necessary.\n\n7. Team Leadership & Staff Support\n\nServe as the Treatment Team Lead for assigned clients.\nProvide day-to-day clinical direction, coaching, and feedback to team members delivering services to those clients.\nIdentify and address performance issues related to clinical documentation, timeliness, or adherence to the ITP and COMAR standards.\nRecognize and affirm staff accomplishments and effective care.\nPromote a collaborative, recovery-oriented, and culturally responsive team culture.\n\n8. Meetings & Supervision\n\nParticipate in weekly clinical supervision when required by license and agency policy.\nMeet weekly with the Operations Manager for caseload review, program updates, and operational coordination.\nMeet weekly with a Human Resources representative to support staffing, training, and compliance matters as they relate to assigned clients and staff.\nMeet weekly with a Finance representative to review billing, productivity, authorization, and payer requirements as they relate to caseload and service delivery.\nAttend all required team meetings, case conferences, and agency trainings.\n\nPHYSICAL DEMANDS: Regularly walk, stand, or stoop; occasionally lift, carry, push, pull, or otherwise move objects weighing up to 10 pounds; and regularly drive a motor vehicle.\nWORK CONDITIONS\n\nCommunity based offices or hybrid setting, with in-person responsibilities based on client needs. \nPossible exposure to emotional or crisis situations with clients\nCollaborative and team-oriented clinical environment\n\nCOMPETENCIES AND SKILLS \n\nPrior experience in outpatient mental health, community mental health, or related behavioral health settings is strongly preferred.Strong skills in clinical assessment, treatment planning, and documentation are required.The ability to lead a multidisciplinary treatment team, provide constructive feedback, and support quality improvement is required.Comfort with electronic medical records and basic computer applications is required.\nReliable transportation is required for occasional travel to community sites where services are rendered.\nStrong organizational skills and the ability to manage time, competing priorities, and regulatory deadlines are required.\nThe ability to coordinate care across multiple providers and systems is required.\nKnowledge of recovery-oriented, person-centered, and trauma-informed care principles is required.\n\nLEVEL OF EDUCATION / TRAINING / QUALIFICATIONS\nEducation\n\nAt least a Bachelor’s degree in psychology, social work, counseling, or a related field (Master’s degree preferred). \n\nMINIMUM QUALIFICATIONS\nLicensure – State of Maryland (one or more of the following):\n\nLicensed Clinical Professional Counselor (LCPC)\nLicensed Graduate Professional Counselor (LGPC)\nLicensed Certified Social Worker (LCSW)\nLicensed Certified Social Worker–Clinical (LCSW-C)\nLicensed Clinical Marriage & Family Therapist (LCMFT)\nLicensed Graduate Marriage & Family Therapist (LGMFT)\nLicensed Master Social Worker (LMSW)\nLicensed Bachelor Social Worker (LBSW)\nLicensed Clinical Alcohol and Drug Counselor (LCADC)\nLicensed Graduate Alcohol and Drug Counselor (LGADC)\nCertified Asso
