Matrix Medical Network seeks NP providers to perform Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities. Each visit lasts up to an hour and covers medical history, medications, and social factors to identify care gaps. You will collaborate with physicians, case managers, and staff to educate patients, order tests, and coordinate follow-up.
