The ideal candidate is compassionate, organized, and committed to helping patients achieve their highest level of function and independence.Essential Duties & ResponsibilitiesEvaluate patients and develop individualized treatment plans based on clinical findingsProvide speech, language, cognitive, voice, and swallowing therapy servicesMonitor patient progress and modify treatment plans as appropriateMaintain accurate, timely, and compliant documentation in accordance with facility, state, and federal guidelinesCollaborate with physicians, nursing staff, therapists, and other interdisciplinary team members to coordinate patient careEducate patients and family members regarding treatment goals, progress, and home strategiesParticipate in care conferences, discharge planning, and interdisciplinary meetingsEnsure compliance with all applicable state, federal, and company policies and proceduresQualificationsGraduate of an accredited Speech Language Pathology programCurrent state Speech Language Pathologist license or eligibility to obtain licensureCertificate of Clinical Competence (CCC-SLP) preferredJoin Our TeamAt Tice Valley Post Acute, you'll be part of a team that values compassion, clinical excellence, and collaboration.
