RN Care Manager: Transitions & Discharge Coordinator

McLaren Health Care in Michigan seeks a full-time Care Management RN to ensure proactive coordination and timely transition of patients to appropriate levels of care. You will work with physicians, nursing, social work, and other team members to optimise LOS, resource use, and readmission prevention. Qualifications include an RN license, a Bachelor’s degree in nursing, and at least three years of acute hospital experience.
