Registered Nurse Home Care
Personal Touch Home Health Care Services Inc.
Role: Registered Nurse providing home-based care for Pediatric and/or Adult patients, including those with g-tubes, tracheostomies, and ventilators. Responsibilities Plan, implement, and evaluate patient care using the nursing process. Initiate and revise care plans based on patient needs and prescriber input. Administer medications and treatments, monitoring their effectiveness. Respond to changes in patient condition and report to physicians or supervisors. Identify opportunities for patient education, care coordination, and discharge planning. Qualifications & Skills Current RN license for the practicing state. Valid health certificate, PPD or chest X-ray, and BLS certification. Preferably, at least one year of RN experience. Additional Information: This role involves working with pediatric and adult patients in home settings, learning and managing complex medical equipment, and providing compassionate, patient-centered care.
