Evaluates, diagnoses, and treats acute and chronic illnesses, injuries, and other health conditions within scope of practice. Develops, implements, and evaluates individualized plans of care in collaboration with patients, physicians, and interdisciplinary team members. Orders, performs, and interprets laboratory, diagnostic, and other tests as appropriate. Prescribes, administers, and monitors medications and therapeutic interventions in accordance with licensure and organizational policies. Provides patient and family education regarding diagnoses, treatment plans, medications, preventive care, and health maintenance. Documents assessments, treatment plans, patient encounters, and clinical outcomes accurately and timely within the medical record. Coordinates patient care services, including consultations, referrals, follow-up care, and transitions across the continuum of care. Collaborates with physicians and other healthcare professionals to ensure safe, effective, and efficient delivery of care. Maintains compliance with applicable regulatory, accreditation, privacy, and organizational requirements. Participates in quality improvement, patient safety, and performance improvement initiatives as assigned. Supports patient advocacy, patient satisfaction, and a culture of compassionate, patient-centered care. Maintains professional competency through continuing education and ongoing professional development. Utilizes sound clinical judgment and evidence-based practice to support quality patient outcomes. Performs other duties as assigned. Knowledge, Skills, and Abilities: Knowledge of patient assessment, diagnosis, treatment, and disease management principles. [Required] Knowledge of applicable healthcare regulations, standards, and scope of practice requirements. [Required] Ability to interpret clinical and diagnostic information and make sound clinical decisions. [Required] Effective verbal, written, and interpersonal communication skills. [Required] Ability to work collaboratively with physicians, patients, families, and interdisciplinary team members. [Required] Proficiency in electronic medical record documentation and clinical systems. [Required] Experience working in a multidisciplinary healthcare environment. [Preferred] Knowledge of quality improvement and evidence-based practice principles.
