We are seeking an experienced Licensed Practical Nurse (LPN) to join a collaborative care team in a field-based Case Manager role. This position is ideal for an LPN who enjoys building relationships with patients and caregivers while providing hands-on clinical support and coordinating care.

The LPN will spend approximately 80% of their time in the field and 20% on administrative responsibilities, managing a caseload of approximately 4-5 patients per day.

Pay: $35-$38/hour
Schedule: Monday-Friday | 32-40 hours/week | Flexible schedule

What You'll Do:

• Manage assigned patient cases and regularly assess patient needs and care plans.
• Provide skilled nursing care, including medication administration, catheter care, and ostomy care.
• Train, mentor, and oversee caregivers on non-skilled patient care, including transfers, repositioning, toileting, and bathing.
• Conduct client introductions and assist with onboarding new patients and care plans.
• Collaborate with case management, scheduling, and operations teams to ensure continuity of care.
• Support community outreach/marketing efforts and assist with general office needs as needed.
• Maintain accurate documentation and communicate changes in patient needs to the appropriate team members.

Requirements:

• Active LPN license in good standing.
• Hands-on experience with medication management and skilled nursing procedures.
• Experience with catheter and ostomy care required.
• Strong communication, judgment, organization, and interpersonal skills.
• Comfortable training and providing guidance to caregivers.
• Reliable work history and ability to work independently in the field.

Preferred:

• 5+ years of LPN experience.
• Previous Home Health experience.

Schedule & Additional Details:

• Monday-Friday, 32-40 hours per week .
• Highly flexible schedule with an expectation of approximately 8 hours per day .
• Weekend on-call patient triage is currently required. Weekend volume can vary, and the long-term goal is to reduce or eliminate the weekend on-call requirement as staffing allows.
• No calls are answered between 10:00 PM and 6:00 AM , and shifts will not begin before 7:00 AM or after 9:00 PM.
• Mileage reimbursement of $0.58/mile for non-commute travel .
• Professional scrubs and closed-toed shoes required.
