Supported Housing Case Manager

Position Full-time Pay $27.65 - $29.70/hour Benefits Generous Paid Time Off – new employees accrue up to 31 paid days off, including 15 flexible use PTO days, 10 paid holidays, 2 personal days, and the final week of the year. Essential Duties and Responsibilities Understands and practices safekeeping of client protected health information per Bridgeways’ HIPAA policies and procedures. Maintain a reliable attendance record and adhere to scheduling requirements based on business and clients’ needs. Provides direct Social and Community Services to facilitate housing placement and post-placement stabilization to clients involved in housing case management services, completing day‑to‑day tasks, paperwork, documentation, and HMIS entries. Provides case management, referral, and supportive counseling using Motivational Interviewing to coordinate with other resources and assist clients in obtaining benefits and documentation. Represents Bridgeways in community and regional activities, committees, and work groups as requested. Establishes liaisons and relationships with appropriate community agencies to meet current and future housing needs. Attends scheduled Sponsoring Agency meetings, maintains knowledge of Shelter Plus Care program requirements and submission of reports. Stays abreast of WA State landlord‑tenant law, screening laws, and other relevant regulations. Specific Tasks Conduct assessments to identify participants’ needs and preferences. Work with participants to develop short and long‑term measurable goals and individualized community integration plans. Provide service‑planning support and participate in person‑centered plan meetings. Coordinate and link participants to community support services, including primary care, behavioral health, and independent living supports. Assist participants with benefit applications, navigation, and monitoring, coordinating with the social service agency. Support participants in developing independent living skills. Coordinate with participants to review, update, and modify housing support and crisis plans. Connect participants to long‑term tenancy resources. Work with private market and scattered‑site landlords to grow and foster housing opportunities. Act as liaison with landlords to promote participant success. Education and Experience Two years’ experience in a social service or behavioral health agency working with adults with chronic and persistent mental illness and familiarity with low‑income housing resources. Lived experience congruent with the target population and/or Peer Certification may substitute for the social service agency requirement. Knowledge of Medicaid, BHO, WAC, and managed care client requirements for clinical services and documentation. Experience in effective provision of clinical behavioral health services in adult mental health. Knowledge of current behavioral health care practices. Knowledge of CQI, quality assurance, and quality improvement processes. Knowledge of local, state, and federal housing guidelines, trends, and funding opportunities; Landlord/Tenant law; Fair Housing laws; HMIS; Shelter Plus Care Guidelines; and community housing resources. Language Skills Ability to read, analyze, and interpret financial reports and legal documents. Ability to respond to common inquiries or complaints from clients, regulatory agencies, or business community members. Ability to understand and contribute to grant requests for housing funding. Mathematical Skills Ability to add, subtract, multiply, and divide using whole numbers, common fractions, and decimals. Reasoning Ability Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to understand broad conceptual terms and specify fine details for developing and implementing programs, policies, and procedures. Certificates, Licenses, and Registrations Valid Washington State Driver’s License. Pass background check in accordance with customer guidelines and requirements. Must be authorized to work in the United States. Other Skills and Abilities Ability to promote Bridgeways housing services in community settings. Ability to learn and utilize electronic medical records for confidential and safe record‑keeping that conforms to agency requirements. Work Environment The employee works in an office space provided in either an administrative office environment or clinical facility environment.
