They have a current opening for an RN Case Manager. The is a hybrid remote/office/field role managing cases for home-bound patients and will oversee nurses and aides. Candidates can be based in and will be required to do field visits in these areas: Rockland Co. NY, Orange Co, NY, and Westchester Co.Home Health (CHHA), Home Care (LHCSA), or Hospice care experience is required.RN Case Manager Job RequirementsLHCSA or CHHA experience.Registered Nurse with NY license.Reliable transportation.Knowledge of NY Medicaid programs including NHTD or TBI Waiver Program preferred.RN Case Manager Job ResponsibilitiesManage case load.Complete assessments and other visits in the field when necessary.Ensure compliance with all state and federal regulations.Oversight of nurses, aides, and C.N.A's.The compensation scale listed is a general salary range and is not intended to represent the actual annual or hourly compensation rate for this role.
