As a Patient Care Coordinator, you will play a crucial role in managing the care and services for patients throughout their treatment journey. You will work closely with patients, families, physicians, and the patient care team to ensure the best possible outcomes. Your advanced clinical skills will be essential in assessing, planning, and evaluating patient care, while also promoting the effective use of resources. You will be a leader within the patient care team, advocating for patients and families, and ensuring seamless communication among providers.

• Coordinate care and services for assigned patients during episodes of illness or treatment
• Utilize advanced clinical skills to assess, plan, intervene, and evaluate patient care
• Promote effective utilization of resources and community services
• Evaluate and analyze quality/utilization cases and criteria
• Collaborate with multidisciplinary teams to ensure efficient and cost-effective care
• Maintain communication with patients and families as a resource
• Initiate clinical pathways based on admission diagnosis
• Need 1 year experience in setting
• Current CA Licensure
• Current CPR recognition
