Social Worker
Transitions Care
Transitions Care is seeking a compassionate Social Worker / LSW Hospice to support patients and families facing serious or terminal illness. In this role, you’ll complete psychosocial assessments, develop person-centered care plans, and provide counseling, crisis intervention, and resource coordination. You’ll collaborate closely with our interdisciplinary hospice and palliative care team to honor patient goals, promote comfort and dignity, and guide families through complex decisions and grief. This position offers professional growth in a mission-driven, values-based environment.
