RN Field Case Manager - Home Health & Care Coordination

UnitedHealth Group is seeking a Field-based Registered Nurse in Seattle, WA to visit members at home, assess health status, identify gaps in care, and educate patients for self-management. You will coordinate with care teams and make referrals to Home Health Aides or Meals on Wheels as needed. The role requires an active WA RN license, BLS or ability to obtain, 1+ year of clinical, home-care experience, and a valid driver’s license with reliable transportation.
