Job DescriptionPrimary Care Physician –Dayton, OH\nCOMPENSATION: Salary $250k plus Incentive Bonus’\n\nOur Primary Care Physician provides equitable and effective value-based healthcare to local Medicare patient populations at our innovative network of neighborhood primary care centers. Our Primary Care Physicians are supported by large care teams so our providers can focus on delivering a better quality of care, rather than a volume of services. Our value-based care model and competitive bonuses are structured to reward outcomes, drive low hospital admissions, deliver preventive medicine, and result in an unmatched patient experience.\n\nRole:\n\n Compensation: Base $230-$250K (wider range available depending on experience and location)\n Bonus: Based on quality metrics\n Schedule: Monday-Friday 8am-5pm\n Patient Volume: 13-16 a day\n Visits: 20 min. regular visit/ 40 min. full diagnostic and new patient\n Physicians care team: Scribe, MA, Social Worker\n Equipment at clinic: Labs on site with a phlebotomist/ access to RubiconMD (gives access to specialist)\n EMR: Canopy and Greenway\n Model: Value Based Health\n Transportation for patients: Van pick up with in 5 miles of clinic\n Community room patient activities: Exercise, events, yoga, dance, spa days\n Providers per clinic: Average 3-5 (mix of NP’s and Primary Care Physicians)\n\n\nBenefits: \n\n Physician Partnership Track\n 6 weeks of PTO, inclusive of PTO, major holidays, and CME\n $5000 Continuing Medical Education stipend\n Provided Health, Vision, Dental, and Life Insurance\n 401K Investment, up to 4% company match, vested immediately\n Provided Medical Malpractice Insurance\n Relocation package on a case-by-case basis\n Sign on: Flexibility for PCP\n\n\nRequired Qualifications: \n\n Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
