MDS Coordinator - RN or LPN

Resolve Harmony Center is seeking an experienced and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as an MDS Coordinator . This position plays a vital role in ensuring accurate assessments, regulatory compliance, quality outcomes, and appropriate reimbursement within our skilled nursing facility. Responsibilities: Complete and coordinate MDS assessments in accordance with state and federal regulations. Ensure accuracy and timeliness of MDS submissions and supporting documentation. Coordinate the interdisciplinary care planning process. Review clinical documentation to ensure appropriate reimbursement and regulatory compliance. Monitor quality measures and identify opportunities for improvement. Collaborate with nursing leadership, therapy, social services, and other departments. Assist with Medicare and Medicaid case management related to resident assessments. Maintain knowledge of current regulations and reimbursement guidelines. Support survey readiness and compliance initiatives. Qualifications: Current RN or LPN license in good standing. Must have a valid AZ Fingerprint Clearance Card Previous MDS Coordinator experience in a skilled nursing facility preferred. Strong understanding of Medicare, Medicaid, PDPM, and MDS regulations. Knowledge of care planning and reimbursement processes. Excellent organizational, communication, and time management skills. Ability to work independently while collaborating with an interdisciplinary team. Strong attention to detail and ability to meet deadlines.
