Licensed Practical Nurse (LPN)

Hourly Rate: $38.00 – $42.00 per hour Location: New Rochelle, NY – Onsite – Long Term Care Facility

Position Summary

As a Licensed Practical Nurse (LPN), you will provide compassionate, high-quality nursing care to residents in a long-term care setting under the supervision of a Registered Nurse (RN). You will support residents' health, comfort, and well-being by delivering patient-centered care while maintaining compliance with clinical standards and facility policies. Responsibilities Schedule / Shift

• Weekend schedule / No night shift
• Option to work every weekend or every other weekend

Key Duties

• Collect and document resident admission information, including medical history and insurance details
• Assist Registered Nurses in providing comprehensive nursing care
• Monitor, record, and report vital signs and changes in resident condition
• Administer medications, injections, and prescribed treatments
• Prepare residents for examinations and collect laboratory specimens
• Provide wound care and treat bedsores as directed
• Assist residents with personal hygiene, dressing, and activities of daily living
• Monitor and document food and fluid intake and output
• Supervise Certified Nursing Assistants (CNAs) as assigned
• Maintain accurate documentation using the PointClickCare electronic medical record system

Requirements Education

• Required: Graduate of an accredited Practical Nursing program

Licenses & Certifications

• Required: Current and active New York State Licensed Practical Nurse (LPN) license
• Current BLS (Basic Life Support) certification
• Up-to-date medical clearance, including COVID-19 vaccination

Skills

• Minimum of 3 months of Long Term Care experience preferred
• Proficiency with the PointClickCare electronic medical record system
• Strong communication and interpersonal skills with residents, families, and healthcare teams
• Excellent assessment, documentation, and critical thinking abilities
• Ability to prioritize tasks, adapt to changing resident needs, and work collaboratively in a fast-paced environment
