Case Manager

Provide social services and assistance to improve the social and integral health of children and their families, to assure family well-being and promote the family self-sufficiency.

ESSENTIAL TASKS AND RESPONSIBILITIES

1. Promote services and recruit children and their families with a two-generation approach that allows the family to achieve individual and family goals.
2. Engage all family members and encourage participation in services to overcome barriers and address needs.
3. Admits new families conducting orientations, reviewing applications and completing intake interviews.
4. Offer case management services; determining individuals and family’s needs, type of intervention and develop plans to improve their well-being.
5. Implements social programs for the achievement of individual and family self-sufficiency.
6. Maintains individual and family records by reviewing case notes; logging events and progress.
7. Communicates individual and family progress by conducting interdisciplinary meetings and evaluations; disseminating results and obstacles to the team and family.
8. Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics and family progress record.
9. Promotes the integration of the two generation approach in all areas of Vimenti Project.
10. Assists and refers the family to other services within and outside the community.
11. Advocates for families rights and supports individuals to acquire the skills and awareness to advocate for themselves.
12. Maintain up to date and supervises the managing of information, documentation and progress reports of individual, family and community interventions.
13. Serve as liaison with community services including social and mental health programs.
14. Refer families to the corresponding agencies according to their particular situations.
15. Keeps up to date in training related to the area of case management and integral development of children, youth and families.
16. Visit agencies and make collaborative agreements with educational and prevention organizations.
17. Manages security and emergency situations, ensuring protocols and organizational policies.
18. Other tasks assigned by your supervisor.

EDUCATION AND/OR EXPERIENCE

BA or Master of Arts with concentration in Social Work, or related field. Minimum of 3 years of experience in social work, behavioral health, or case management, or education or certifications, or equivalent experience. Social Work Professional License required.

KNOWLEDGE SKILLS AND ABILITIES

Excellent interpersonal and communication/listening skills, including written and oral skills in both English and Spanish preferred or when feasible. Demonstrated proactive orientation with emphasis in academic and leadership credibility. Ability to build and manage relationships with community-based and public agencies supporting care coordination. Computer/technological skills; ability to research and abstract relevant educational developments, from the Web. Ability to organize, prioritize, and manage time well. Proficient in adapting to changing situations and efficiently alternating focus between tasks to support the operations as dictated by business needs.

PHISICAL DEMANDS

Individuals may need to sit or stand as needed. The position may require walking primarily on a level surface for periodic periods throughout the day. May reach above shoulder heights or below the waist and lift as required to file documents or store materials throughout the workday and must be able to lift 15 pounds and use proper lifting techniques.

MENTAL DEMANDS

Observing, receiving, and otherwise obtaining information from all relevant sources. Identifying the underlying principles, reasons, or facts of information by breaking down information or data into separate parts.
