Alignment Health is seeking a compassionate, motivated Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members throughout Maricopa County, Arizona. In this highly field-based role, you'll build trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits, access community resources, and improve their overall well-being. If you enjoy working independently, connecting with people face-to-face, and making a direct impact in your community, we'd love to meet you.What You'll DoConduct approximately 6-8 in-home visits daily throughout your assigned service area.Provide wellness coaching and health education to support behavior change and healthier lifestyles.Partner with Advanced Practice Clinicians and Social Workers to develop member action plans.Connect members with community resources and social services.Support member engagement, retention, and satisfaction.Coordinate care with internal clinical teams and external partners.Deliver health education materials to members, caregivers, and families.What to ExpectPrimarily field-based role with daily local travel.Monday-Friday, 8:00 AM-5:00 PM PST.Visits are scheduled geographically to maximize efficiency.Mileage reimbursement provided from door-to-door travel.Minimum Requirements:Bilingual Spanish, must be able to write, read, speakMinimum 1+ year of experience as a wellness health coach or educatorCurrent, valid, unrestricted California Driver's License.Reliable personal transportation and ability to travel throughout the assigned territory.Ability to maintain automobile insurance in accordance with company requirements.Ability to work independently and remotely.Strong communication, organizational, time management, and relationship-building skills.Proficiency with Microsoft Office (Excel, Word, and PowerPoint).Knowledge of health, wellness, nutrition, exercise, and behavior change principles.Ability to maintain HIPAA compliance and confidentiality.Education:Associate's degree or two years additional experience in lieu of education.Preferred Requirements:Senior care experienceBachelor's degreeMedical Assistant LicenseWhy Alignment Health?Meaningful, member-focused work that improves lives every day.Independence and flexibility in a community-based role.Opportunity to build lasting relationships with members and families.Collaborative, mission-driven care team dedicated to better health outcomes.Work EnvironmentThe work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
