DEMENTIA COORDINATOR

Job Location: Newark, DE 19702. Delaware Hospice is actively seeking a full‑time Dementia Care Coordinator. Schedule will be Monday‑Friday 8am‑430pm. The Dementia Care Coordinator plays a key role in supporting the team in delivery of care coordination, respite, education, and resources to support people living with dementia, their families, and caregivers. This role would best be filled by someone that is compassionate, detail oriented, has strong communication and organizational skills, and can multi‑task and prioritize independently.
