An opportunity is available for a Pharmacist II – Clinical Pharmacist to join a dynamic hospital team located in Utica, NY. This direct hire position offers the chance to play a vital role in delivering clinical pharmacy services within a decentralized setting, including emergency departments and intensive care units. Reporting to the Pharmacy Operations Manager, Clinical Pharmacy Manager, or designee, the role is designed for pharmacists with a passion for optimizing patient outcomes and advancing pharmacy education.
Schedule:
- 7 days on, 7 days off, night shifts
- Shift differential applies
Responsibilities:
- Provide comprehensive clinical pharmacy services in assigned hospital units
- Collaborate with healthcare providers to ensure optimal medication therapy management
- Review and verify medication orders for appropriateness, dosing, interactions, and allergies
- Participate in multidisciplinary rounds and offer expert recommendations
- Conduct medication reconciliation, patient counseling, and handle drug information inquiries
- Monitor patient medication profiles to identify and manage drug-related problems
- Support the PGY1 pharmacy residency program through precepting and education
- Prepare sterile IV admixtures and compounded sterile products following USP standards
- Train and oversee pharmacy technicians and staff involved in sterile compounding
- Mentor pharmacy students and contribute to clinical education initiatives
- Engage in medication safety, quality improvement, and policy development efforts
- Ensure compliance with New York State and federal pharmacy regulations
Qualifications:
- Doctor of Pharmacy (PharmD) degree from an accredited institution
- Active pharmacist license in New York State, in good standing
- 1–3 years of hospital pharmacy experience with clinical expertise in specialty units
- Completion of a PGY1 pharmacy residency required
- Board certification through the Board of Pharmacy Specialties (BPS) preferred
- Experience precepting pharmacy residents or students is a strong asset
- Demonstrated interest in clinical education and pharmacist mentoring
