Pediatric RN Case Manager Field-Based Care Coordinator

Cook Children's Health Care System is seeking a case manager to perform coordination activities that help members manage chronic conditions. This office-based position requires field visits, accounting for up to 75% of the workweek. Candidates should hold a current RN license in Texas and possess strong critical thinking and communication skills. Preferred qualifications include three years of clinical experience and familiarity with vulnerable populations.
