Make a meaningful difference with flexible scheduling and focused patient care. We are seeking a compassionate Hospice Nurse Practitioner to complete Medicare required FacetoFace (F2F) visits for hospice recertification. This role offers autonomy, supportive leadership, and the opportunity to focus on what matters most—patients and families. Responsibilities: Conduct hospice FacetoFace visits in homes and facilities Perform clinical assessments supporting continued hospice eligibility Complete accurate, timely documentation in the EMR Collaborate with physicians and interdisciplinary team members Qualifications: Active Nurse Practitioner license Master’s or Doctorate in Nursing National NP certification (ANCC/AANP) Hospice, palliative, or geriatric experience preferred Strong assessment and documentation skills What We Offer: Flexible scheduling (FT / PT / PRN) Minimal oncall requirements Competitive compensation Supportive, missiondriven hospice team Apply today to join a team committed to comfort, dignity, and compassionate care.
