Work Environment

Key Responsibilities

• Conduct thorough assessments to discern clients’ social, emotional, and practical needs, implementing individualized service plans that address those challenges.
• Facilitate counseling sessions for individuals and families coping with grief, loss, and other end-of-life issues, providing a safe space for emotional expression and support.
• Develop and monitor personalized service plans that include specific goals, interventions, and measurable outcomes to track client progress.
• Collaborate with clients to identify and connect them with appropriate community resources, ensuring access to essential services such as financial aid, counseling, and housing support.
• Engage in crisis intervention and offer risk assessments, conducting safety planning when necessary to protect client welfare.
• Foster strong partnerships with community organizations and healthcare providers to coordinate care and advocate for client needs effectively.
• Participate in case conferences and regular team meetings to discuss client progress and share insights with colleagues.
• Conduct community outreach efforts to educate the public on available services and reduce stigma around seeking help.

Patient Population / Client Focus

Your work will primarily focus on individuals and families who are navigating the complexities of hospice care, often experiencing profound emotional distress as they deal with terminal illness and loss. You will support diverse clientele, ensuring culturally competent care that respects the varied backgrounds and circumstances of each client. Qualifications

• A Master’s degree in Social Work from a recognized and accredited program.
• Current active licensure as a Licensed Master Social Worker (LMSW) in the state of Nevada.
• Strong understanding of social, economic, and health issues affecting hospice clients.
• Excellent communication and interpersonal skills, enabling collaboration with diverse populations.
• Strong analytical and problem-solving abilities to tailor interventions to client needs.

Preferred Experience

While a minimum of one year of experience in a social service agency is required, preferred qualifications may include experience in hospice, palliative care, or community mental health settings. Familiarity with trauma-informed care principles and knowledge of community resources is highly desirable.

Compensation and Benefits
