Key Responsibilities

• Conduct thorough assessments of patients' psychosocial needs to formulate individualized treatment plans that reflect their values, beliefs, and cultural backgrounds.
• Provide evidence-based therapeutic interventions, such as cognitive-behavioral therapy and support groups that foster resilience and coping strategies in patients and their families.
• Maintain meticulous clinical documentation in accordance with regulatory requirements and organizational standards, ensuring patient confidentiality while utilizing electronic health records (EHR) systems.
• Actively participate in case conferences with an interdisciplinary team, contributing insights and recommendations to enhance holistic patient care.

Patient Population / Client Focus

The LCSW will primarily work with hospice patients who are experiencing acute grief, emotional distress, and complex psychosocial issues. Qualifications

• A Master’s degree in Social Work from an accredited institution.
• An active LCSW license in California.

Compensation and Benefits

The Family Health Centers offers a competitive salary ranging from USD 120,000 to 130,000 per year , commensurate with experience and qualifications.
