Work Environment

This role is primarily based onsite, visiting patients in their homes across Palm Desert and nearby communities. This unique work setting allows therapists to engage with patients in familiar surroundings, which has been shown to enhance motivation and facilitate recovery. You will utilize your physical assessment skills and therapeutic techniques to create a warm and supportive atmosphere conducive to healing. Fortis Motion LLC values the well-being of our staff, offering flexibility and support within our scheduling framework, typically operating Monday through Friday from 9 AM to 5 PM.

Key Responsibilities

• Deliver individualized, evidence-based treatments to patients with a variety of orthopedic, post-surgical, sports-related, and neurological conditions.
• Conduct thorough evaluations to design customized treatment plans that address the specific rehabilitation goals and needs of each patient.
• Maintain ongoing communication with referring physicians and interdisciplinary care teams to ensure coordinated and effective patient outcomes.
• Educate patients and their families about their conditions, rehabilitation processes, and provide them with tools to promote long-lasting recovery.
• Document patient progress meticulously in compliance with healthcare regulations and standards, utilizing electronic health record (EHR) software.
• Participate in regular team meetings to discuss patient care strategies, share insights, and collaborate on improving treatment protocols.

Patient Population / Client Focus

Your primary focus will be on outpatient clients requiring physical therapy within their homes. This patient population may include individuals recovering from surgeries, managing chronic conditions, or seeking rehabilitation following injury or illness. By providing one-on-one care, you will have the opportunity to foster genuine relationships with your patients, understanding their unique challenges and supporting them through every step of their recovery process.

Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• A minimum of two years of clinical experience in a related field is essential, demonstrating increasing responsibility and skill development.
• Current California Physical Therapy License or the ability to obtain one before the start date.
• Strong interpersonal and communication skills, with the ability to listen actively and interact empathetically with patients and families.
• A commitment to professional growth, continuous learning, and team-based care.

Preferred Experience

• Experience in outpatient orthopedic or sports rehabilitation is highly preferred.
• Familiarity with relevant EHR systems and documentation practices is beneficial.

Compensation and Benefits
