Must-Have: Prior acute care coordination experience. Experience with discharge planning and utilization review. Proficiency in using Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs. Familiarity with Medicare/Medi-Cal guidelines related to SNF and lower-level placements. Experience working with placement agencies and navigating heavily socially-impacted discharges.

Description: Seeking an experienced RN for a Case Management role. The role involves care coordination and case management for patients in an acute care setting. Ideal candidates must be adaptable, flexible, and able to hit the floor running. Candidates are expected to maintain excellent attendance and take scheduled breaks. The position requires covering all adult areas of the hospital and may involve floating when needed. A clean, professional appearance is necessary, with specific dress code requirements. Candidates should have experience managing complex discharges for patients with limited resources. Strong knowledge of strategies for successful discharges is preferred. The work environment is dynamic, and staff will engage with various patient needs, including room and board placements.
