RN Case Manager (Denton County)

Visiting Nurse Association of Texas

Job Description

Job Description

Make a Meaningful Difference in the Places That Matter Most. Are you a dedicated RN with a heart for hospice care? We are looking for a compassionate Registered Nurse to join our interdisciplinary team, providing skilled nursing care to patients in the comfort of their own homes. If you are passionate about supporting patients and their families through life’s most delicate transitions, we invite you to bring your clinical expertise to a role where your empathy is as valued as your skills. This role is Monday-Friday 8:00 AM -5:00 PM in Dallas County. Primary functions are to administer nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care.

Established in 1934, the Visiting Nurse Association of Texas (VNA) is a non-profit organization providing compassionate care to the community. As the first and most experienced Hospice Care provider in Texas, VNA has a trusted reputation for delivering high-quality care for over 90 years. Our hospice and palliative care programs are available across 16 counties in North Texas, including: Collin, Cooke, Dallas, Denton, Ellis, Fannin, Grayson, Henderson, Hunt, Johnson, Kaufman, Rockwall, Tarrant, Van Zandt, and parts of Wise and Navarro Counties.

Joining our team means more than a competitive salary and mileage reimbursement. We offer comprehensive medical, dental, and vision plans, along with employer-paid life and short and long-term disability insurance. To support your future, we offer a 401(k) with a 6% match and a fast, two-year vesting schedule. We also prioritize work-life harmony through generous PTO, eight total paid holidays, and opportunities for professional growth and tuition reimbursement—all within a supportive, mission-driven culture.
