• Salary: $150,000–$160,000 per year (Based on experience) + Benefits

Position Overview

We are seeking an experienced, dedicated Nurse Practitioner (NP) to provide high-quality, patient-centered primary care to residents of assisted living facilities throughout Queens, New York.

We are looking for a provider who combines strong clinical judgment with outstanding communication and patient service. Our approach emphasizes accessible, attentive care, prompt communication, and dependable follow-through to ensure an exceptional experience for residents, families, and facility staff.

-> Key Responsibilities

• Provide comprehensive primary care, including evaluation, diagnosis, and treatment of acute and chronic medical conditions.
• Conduct routine assessments, preventive screenings, wellness visits, and follow-up appointments.
• Prescribe and manage medications within applicable scope-of-practice requirements.
• Review laboratory results, imaging studies, and other diagnostic findings.
• Coordinate care with specialists, facility staff, family members, and other healthcare professionals.
• Respond promptly to clinical concerns, patient needs, and facility communications.
• Maintain accurate, thorough, and timely documentation in the electronic medical record.
• Demonstrate professionalism, punctuality, accountability, and strong communication skills.
• Build positive, collaborative relationships with residents, families, and assisted living facility teams.
• Promote continuity of care through consistent follow-up and effective care coordination.

-> Required Qualifications

• Active, unrestricted New York State Nurse Practitioner certification and Registered Nurse (RN) license.
• Minimum of three years of primary care experience as a Nurse Practitioner.
• Active DEA registration, as applicable to the position.
• National Provider Identifier (NPI).
• Active Medicare and Medicaid enrollment, as applicable.
• Strong clinical assessment, diagnostic, and medication management skills.
• Excellent interpersonal, organizational, and time-management skills.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Commitment to compassionate care, timely documentation, and reliable follow-through.

-> Preferred Qualifications

• Experience caring for geriatric patients.
• Experience working in assisted living, long-term care, or similar healthcare settings.
• Familiarity with eClinicalWorks (eCW).
• Experience coordinating care with facility administrators, nursing teams, and specialists.
