Job Title: Home Health Physical Therapist (PT)
Location: Seaside, OR
Duration - 13 Weeks
Job OverviewWe are seeking an experienced Home Health Physical Therapist (PT) to provide skilled therapy services to patients in their homes. This role is ideal for a therapist who enjoys working independently, managing a caseload, and helping patients improve mobility, function, and quality of life in a home-based setting.
Schedule- 8-Hour Day Shifts
- No Weekends
- No On-Call
- Active Physical Therapist License (pending license accepted)
- BCLS Certification required
- Minimum 1 year of Skilled Home Health experience required
- OASIS experience required
- First-time travelers accepted
- Reliable personal transportation required
- Perform comprehensive patient evaluations in the home setting.
- Develop and implement individualized treatment plans.
- Provide therapeutic interventions to improve strength, mobility, balance, and functional independence.
- Complete OASIS assessments and required home health documentation.
- Educate patients and caregivers on treatment plans, safety, and home exercise programs.
- Coordinate care with physicians, nurses, and interdisciplinary healthcare teams.
- Maintain accurate and timely documentation using the agency's electronic medical record system.
