Qualified Intellectual Disability Professional (QIDP) / Case Manager Location: Community Day Services Program, Tinley Park, IL. Job Type: Full Time, Monday to Friday, no holidays or weekends. Position Overview Schedule: Monday to Friday days, flexible hybrid (in-person and remote). Responsibilities Develop and implement individualized strategies based on each person's goals, preferences, and personal plan. Coordinate services by maintaining regular contact with individuals on your caseload. Participate in discovery and interdisciplinary team meetings. Complete required assessments, documentation, and consents. Monitor progress and outcomes through monthly reviews and reporting. Compensation & Benefits Competitive pay: $25 per hour.
