- Conduct thorough evaluations of patients in both inpatient and outpatient environments, tailoring individualized treatment plans for optimal results
- Implement physical therapy interventions with a focus on enhancing mobility, strength, balance, and functional capabilities
- Collaborate closely with nurses, physicians, and other rehabilitation professionals to ensure comprehensive care and the best patient outcomes
- Educate patients and their families about therapeutic objectives, discharge processes, and exercises to continue at home
- Document treatment processes, evaluations, and patient progress in strict accordance with facility standards and regulatory guidelines
- Engage in interdisciplinary rounds and conferences to ensure a holistic and coordinated approach to patient care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program
- Active state licensure as a Physical Therapist
- CPR certification is preferred
- Additional specialized certifications (e.g., in neurology or geriatrics) are advantageous, but not essential
- Previous clinical experience is highly preferred
- Proficiency in treating patients with complex medical challenges and mobility concerns
- Excellent communication and teamwork skills are essential
- Comfort with digital charting and clinical documentation systems
