An employer is hiring remote RN case managers for a healthcare startup redefining how care is delivered for patients with neurological and endocrine conditions. This is a fully remote role where you'll play a key part in helping patients navigate complex, chronic conditions while contributing to a rapidly scaling care model.

What you'll do:

• Build ongoing relationships with patients through monthly virtual check-ins to track progress and adjust care plans
• Support patients with neurological (Alzheimer's, Parkinson's, MS) and endocrine conditions (diabetes, thyroid disorders, metabolic conditions)
• Coordinate care logistics including referrals, appointment scheduling, and medication reminders
• Identify and recommend personalized resources (nutrition, exercise, sleep, lifestyle) tailored to each patient's condition
• Partner with caregivers and clinical teams to ensure a seamless, supportive care experience
• Educate patients on symptoms, disease progression, and modifiable risk factors in a clear, approachable way

• Mentor and train new LVNs and RNs
• Support clinical escalations and decision-making
• Collaborate directly with leadership (CEO & Medical Director) to improve care models and protocols
