The Post‑Acute Care Team improves the quality of care and reduces cost by safely transitioning patients back to the community. Responsibilities Population Health Clinical Coordinator (PT/OT) evaluates post‑acute care services for the MSHP population, monitors patient outcomes, and ensures timely discharge from skilled nursing facilities: Identify MSHP lives served in SNFs daily Conduct patient reviews, assess care plans, and reinforce goal LOS Discuss potential discharge needs and barriers Ensure SNF providers implement care pathways and best practices Reinforce quality and evidence‑based care to reduce cost and optimize outcomes: Participate in clinical rounds and review care plans with providers Identify progress toward desired outcomes and intervene to address deviations Generate quality‑improvement results Refer cases with quality concerns to the Senior Director of the Care Continuum Identify potentially unnecessary services and recommend alternatives: Act as liaison between MSHP and SNFs Participate in discharge planning Focus on inappropriate utilization while ensuring appropriate care Monitor and evaluate services and resources: Oversee data collection process Review utilization patterns; identify trends and problems Qualifications Education and Experience Graduate of an accredited Physical Therapy / Occupational Therapy program Current New York State PT/OT license Minimum five years experience in skilled nursing facilities and/or acute care settings Experience in utilization management is a plus Additional Skills and Qualities Excellent relationship‑management skills and assertiveness Strong problem‑solving, organizational skills, and attention to detail Computer literacy in EMR and Microsoft Office suite Effective communicator, both written and verbal, with medical staff and management Consistent record of meeting timelines and objectives Fluency in English Equal Opportunity Employer The Mount Sinai Health System is an equal opportunity employer, complying with all applicable federal civil rights laws.
