Key Responsibilities

• Engage and enroll community members eligible for ECM per care plan protocols
• Assess client need areas
• Develop an individualized care plan with each client that is updated as needed
• Coordinate care for clients with a multidisciplinary clinical team and outside agencies
• Facilitate referral linkages to community resources and perform case management services
• Assist clients with the development of social and community support systems
• Develop effective working relationships with agencies, organizations, and providers to advocate for clients
• Document and maintain clinical records per appropriate standards and regulations
• Conduct outreach in diverse community settings
• Participate in clinical supervision, consultations, and trainings
• Provide translation services as needed
• Complete other duties and related projects as assigned

Required Qualifications

• 1+ years of experience
• Bachelor's Degree in Psychology or a related field, or equivalent training and experience
• Valid California Driver License with current personal auto insurance and insurable MVR
• English
