About the Position

Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year License: Current licensure in the state of Massachusetts Certifications: None specified Must-Have: 1 year Certified Home Care experience 1 year Case Management Experience Reliable, independent means of transportation to make home visits Willingness to work weekends, holidays, and on-call as needed Weekend rotations once every 6 weeks Description: Providing clinical services to patients from birth to over 100 years in their homes. Assessing patient conditions and implementing care plans. Offering ongoing education and support to patients and their families. Conducting the admissions process and managing cases in accordance with established policies and standards of practice. Utilizing thorough and timely electronic documentation and interdisciplinary communications.
