Registered Nurse Home Health

Job Summary:


The RN Nurse Assessor in Home Health is responsible for conducting comprehensive assessments in patients’ homes to evaluate their health status, care needs, and preferences. This role involves working both in the field and via telehealth to develop individualized long-term care plans and ensure seamless service delivery.

Responsibilities:

• Perform initial in-home evaluations to assess patient's health, strengths, and care requirements.
• Review medical documentation and health referral forms relevant to each case.
• Enter evaluation data accurately into electronic forms and transmit as needed.
• Document concerns, conflicting information, and issues identified during assessments.
• Foster continuity of care by reducing fragmentation and gaps in treatment plans.
• Communicate with consumers about healthcare options, support services, and waiver programs.
• Initiate and coordinate enrollment processes with the call center and eligible consumers.
• Collaborate with interdisciplinary care team members to enhance care coordination.
• Collect and record quality review data, case notes, and outcome measurements.
• Identify opportunities for health promotion and preventive care.
• Maintain knowledge of community resources, payer requirements, and network services.
• Adhere to all standards outlined in performance criteria and project guidelines.
• Perform additional duties as assigned by management.

Qualifications:

• Current RN licensure in New York State.
• Minimum of 2 years’ recent experience in a home health setting within the last ten years.
• Experience conducting assessments similar to UAS or OASIS procedures.
• Ability to perform in-home evaluations and use technology for data entry.
• Strong organizational, interpersonal, and communication skills.
• Demonstrated sensitivity to patients' needs and effective multi-generational communication.
• Ability to work independently in a fast-paced environment.
• Experience with hybrid telehealth and in-field assessments.
• Preferred: UAS-NY assessment experience, geriatric care, and chronic disease management.
