Community Health Navigator & Case Manager
Union-Community-Care
Union Community Care seeks a Community Health Resource Coordinator to help patients meet social and behavioral goals. You will work with medical, dental, and behavioral teams to coordinate social services and enroll families in health insurance programs, providing language-accessible support and advocacy across community partners. You will document activities in the EHR, engage at outreach events, and collaborate to remove barriers to care, promoting equity and whole-health outcomes for #J-18808-Ljbffr Union-Community-Care
