Key Responsibilities

• Conduct thorough Physical Therapy evaluations for patients, incorporating clinical assessments to establish personalized treatment plans.
• Develop, implement, and modify individualized treatment protocols based on patient needs and progress toward rehabilitation goals.
• Utilize evidence-based practice to regularly assess and document patient responses, modifications to treatment, and ongoing patient progress through comprehensive SOAP notes and other reporting tools.
• Educate patients and their families on rehabilitation processes, potential challenges, and strategies for effective recovery.
• Achieve minimum utilization goals and contribute to departmental objectives through actively engaging in professional development and in-service training sessions.
• Collaborate effectively with multidisciplinary teams to coordinate and optimize care plans for patients receiving multiple therapeutic services.

Patient Population / Client Focus

Qualifications

• A Bachelor's Degree in Physical Therapy; a Master's or Doctor of Physical Therapy (DPT) degree from an accredited program is required.
• A current and unrestricted Physical Therapy license in the state of California.
