Licensed Practical Nurse (LPN) – Full-Time Home Health | Albuquerque & Rio Rancho, NM

Responsibilities

·         Provide direct patient care and implement RN care plans 
·         Document visits accurately and timely 
·         Educate patients and caregivers
·         Communicate changes in condition to RN
·         Follow physician orders and company policies 

Qualifications

·         Active NM LPN license
·         Current CPR certification 
·         Valid driver’s license & reliable transportation
·         Strong communication and documentation skills

Schedule

·         Travel within Albuquerque & Rio Rancho
