Physical Therapist Assistant (PTA) - PRNGreenridge Post-Acute and Senior Living is hiring a licensed Physical Therapist Assistant on a PRN basis. This position is classified as Non-Exempt and reports to the Director of Rehabilitation, the supervising Physical Therapist, and the Facility Administrator.Core responsibilities include:Implementing resident treatment plans in alignment with the established plan of careDelivering skilled therapeutic interventions targeting strength, mobility, balance, coordination, endurance, and functional performanceMonitoring, documenting, and reporting resident responses and any clinical changes to the supervising Physical TherapistEducating residents and caregivers on home exercise programs and safety strategies as delegatedCompleting accurate, timely, and compliant clinical documentation consistent with federal, state, payer, and organizational requirementsMaintaining adherence to HIPAA, infection control standards, OSHA regulations, patient rights, and all applicable California state practice standardsSupporting discharge planning and transition-of-care activities as directedParticipating in team meetings, care conferences, and quality assurance initiatives as requestedTreating all residents with dignity, respect, and compassionate professionalismPhysical requirements include the ability to assist with patient transfers, mobility support, repositioning, prolonged standing and walking, bending, reaching, and lifting with or without assistive devices. The ability to assist in resident evacuation during emergency situations is also required.What You Bring:Current, active, and unrestricted Physical Therapist Assistant license in the State of CaliforniaGraduate of an accredited Physical Therapist Assistant program — requiredSuccessful completion of the NPTE for PTAs — requiredCurrent CPR/BLS certification (or willingness to obtain per facility policy)Excellent verbal and written communication skillsComposure and professionalism in dynamic, fast-paced clinical environmentsStrong organizational skills and the ability to manage competing priorities independentlyA team-first mindset and a genuine passion for skilled nursing and post-acute careEqual Opportunity and AccommodationGreenridge Post-Acute and Senior Living is proud to be an Equal Opportunity Employer.
