Working collaboratively with the interdisciplinary team, the role addresses social, emotional, and practical barriers to care and connects patients with appropriate community resources. Screen patients for social work needs and complete psychosocial assessments as needed. Provide counseling, care coordination, and transition planning. Collaborate with the care team to support safe, timely discharge and optimal outcomes. Connect patients and families to community and referral resources. Assist with family meetings, guardianship, abuse/neglect concerns, psychiatric referrals, end-of-life issues, and charity care. Participate in rounds, quality improvement, supervision, and on-call/coverage duties as needed. Maryland LMSW required Healthcare or field placement experience preferred Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
