What You’ll Do Provide high-quality, one-on-one physical therapy to geriatric patients in their homes. Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge). Create and implement effective plans of care (POC) to improve patient mobility and safety. Coordinate seamlessly with our nursing and clinical leadership team. What You Need Active Florida Physical Therapist (PT) License. Current CPR/BLS Certification. Valid Driver’s License and reliable transportation.
