Key Responsibilities

• Conduct thorough evaluations of patients, including gathering medical histories, performing specific assessments, and formulating detailed care plans based on evaluation findings.
• Clearly communicate evaluation results and treatment options to patients and their families, engaging them in the goal-setting process for recovery.
• Implement and demonstrate proficiency in therapeutic techniques and treatment protocols in alignment with both patient needs and current healthcare guidelines.
• Collaborate with interdisciplinary teams, consulting with healthcare professionals as required and documenting any changes in patient conditions to optimize care plans.
• Manage discharge planning and ensure appropriate follow-up procedures are in place, providing accurate documentation of patient services, including progress notes and discharge summaries.
• Exhibit effective time management and productivity, maintaining accurate schedules while prioritizing patient care and clinic responsibilities.

Qualifications
